After all this time has the Independent finally got to the bottom of the horror of the Hillsborough disaster.

In further revelations on page 6 it seems that both the Fukashima and Chernobyl melt downs were due to engineers and technicians refusing to carry out essential maintenance in their pants and vests when they'd forgotten their overalls.
